These Terms and Conditions ("Terms") govern your use of the website and services provided by Greene Roots Wellness & Telehealth PLLC ("the Practice," "we," "us," or "our"). By accessing or using our website and services, you agree to comply with and be bound by these Terms.

SECTION 1: USE OF WEBSITE AND SERVICES

1.1 Scope of Services

The Practice provides primary care services (for ages 13 to geriatric), medication management, post-procedure care, and concierge services delivered via a hybrid model of telehealth and in-home house calls. All services are rendered by authorized healthcare providers (Nurse Practitioners).

1.2 Appointment and Service Terms

Appointments can be scheduled online. Cancellations should be made at least as much time in advance as noted on the website (i.e., usually at least 2-24 hours in advance). 

Failure to attend scheduled appointments without prior cancellation may result in fees or service restrictions up to or including dismissal from the Practice and/or any or all services offered.

We reserve the right to institute a cancellation fee policy at any time and in any amount we deem fit for the level of service requested at the time of making the appointment.

1.3 Not for Emergencies

DO NOT use this website or our telehealth services for medical emergencies. If you are experiencing a medical emergency, call 911 immediately or proceed to the nearest emergency room.

1.4 Telehealth Disclaimer

You understand and agree that telehealth services are not a substitute for in-person care in all circumstances. The care provided via telehealth relies on the information and technical quality you provide.

SECTION 2: PATIENT CONDUCT AND ACCOUNT

2.1 Eligibility

By using our services, you confirm you are at least 18 years of age or possess the necessary legal consent to receive healthcare services.

2.2 Account Security

You are responsible for providing accurate, current, and complete information when using our services. You are responsible for maintaining the confidentiality of your scheduling and patient portal login credentials (if applicable) and for all activities that occur under your account. You agree to notify us immediately of any unauthorized use. You agree to comply with all applicable healthcare and privacy laws.

2.3 Prohibited Use

You agree not to use the website for any unlawful purpose, including uploading harmful code, transmitting PHI via non-secure methods (e.g., website contact forms), or infringing upon the intellectual property rights of the Practice or others.

SECTION 3: FINANCIAL AND BILLING

3.1 Payment Responsibility

You understand and agree that you are personally responsible for all charges for services provided by the Practice. This includes any co-payments, deductibles, or non-covered services not paid by your insurance provider.

3.2 Billing and Insurance

We will submit claims to your insurance provider on your behalf for covered services. You agree to provide accurate insurance and billing information and to promptly pay any balances due after insurance processing.

SECTION 4: PRIVACY AND CONFIDENTIALITY

4.1 Governing Policy

The collection, use, and protection of your data, including PHI and website-collected data, are governed by our separate Privacy Policy and Notice of Privacy Practices, which is incorporated by reference into these Terms. By agreeing to these Terms, you also acknowledge you have reviewed and agree to the Privacy Policy.

4.2 Data Collection Disclaimer

You acknowledge that while our specific patient forms (Acuity/FormDr) are secure and HIPAA-compliant, the general website communication channels (e.g., chat boxes, non-patient-specific inquiry forms) are not for transmitting Protected Health Information (PHI).

SECTION 5: INTELLECTUAL PROPERTY AND COPYRIGHT

5.1 Ownership

The content, design, logo, and original materials on the website (www.greenerootswellness.org, www.greenerootswellness.com, www.greenerootswellness.net ) are the property of Greene Roots Wellness & Telehealth PLLC and are protected by U.S. and international copyright laws.

5.2 Limited License

You are granted a limited, non-exclusive, non-transferable license to access and use the website for your personal, non-commercial use only. You may not reproduce, redistribute, or commercially exploit any content without express written permission from the Practice.
